ABOUT LADC
Administration and Organization
The Life Center is licensed by the Oklahoma Department of Health. It
is a non-profit organization and is governed by a voluntary Board of
Directors comprised of local, concerned citizens.
Funding
Funding sources include private fees, the Oklahoma Department of Human
Services, Medicaid Advantage Waiver, the Stillwater Area United Way, and
donations.
Fees
Fees are based on a set daily and hourly rate. Persons may be eligible
for partial or full coverage for services provided through the Oklahoma
Department of Human Services, the Medicaid ADvantage Waiver program, or
Center scholarship funds.
Hours of Operation
The Life Center is open Mondays through Fridays from 7:30 A.M. to 5:30
P.M. As space permits, Participants may arrange to attend all or part
of any day or week or attend on a “drop-in” basis.
Location
The Life Center is located in the educational building of the First
Christian Church at 411 W. Mathews in Stillwater, Oklahoma.
Staff
The Life Center staff includes a Program Director, a Health Services
Coordinator, an Outreach and Caregiver Support Coordinator, an Activity
Director, a Dietary Supervisor, Physical and Recreation Therapists, a
Licensed Nurse, Certified Nurse Aides, and Program Assistants.
